What is the HRA tool?
It is an online tool you can use to get a picture of your overall health and how your lifestyle choices are affecting your health. The goal is to help you lead a healthier life.
From October 1, 2009 until December 31, 2009 you’ll have the chance to participate in the Feel Alive contest. By updating and/or completing your Health Risk Assessment, you will be eligible for up to three gift cards, two valued at $300 for October, November, and $400 dollars in December!

How does the HRA tool work?
The HRA tool will ask you to enter the 10-digit policy number found on your Blue Cross ID card. This number identifies the organization you work for only. It provides no personal, identifying information about you.
Next complete the confidential online questionnaire about your diet, exercise habits, stress, your family health history, and other areas related to your health. The whole process will take between 20 and 30 minutes.
When you've completed the questionnaire, your results are immediately compiled into a "personal profile" and you are given a "health risk score."
Your personal profile looks at the behaviours that contribute to your risk score and suggests changes. This is confidential information, password protected by you, and can only be accessed by you.
How do I enhance the valuable information the HRA provides?
As an enhancement to the valuable information this online tool provides, plan members also have the option to enter the results of the following quantitative clinical measurements in order to obtain a more individualized risk assessment:

Blood Pressure

Cholesterol Levels-- total cholesterol, HDL and triglycerides

Blood Glucose Level

Other measurements used in HealthChoice calculations: waist and hip circumference, height and weight.
Coverage for Clinical Measurement Services was added to the Extended Health Plan effective October 1, 2004. The coverage is available to plan members only, not to their spouses or dependents. Clinical Measurement Services must be provided by a doctor or registered nurse and there is a yearly maximum of $100 per person for these services. Here's what you will need: explanatory letter to health care provider and a claim form to obtain reimbursement.
